Monroe County Clerk’s Office Mobile DMV

Our mobile DMV unit is able to provide many basic DMV services, including processing requests for non-driver identification cards.  We can also provide forms, information, and “rain checks” for those who want to get a non-driver ID but don’t have all the required forms of ID with them on the day of the event.  To complete an application, individuals need to have either had an ID issued in the past (after 2003) or have their birth certificate and social security card in hand. booth 55

Social Security Administration

We will be available to set up appointments and take applications for replacement Social Security cards. We will also be able to answer general questions about benefits or speak to someone about their specific case. booth 4

Monroe County Vital Records

If you were born in Monroe County, you can get a birth certificate on the spot. To be eligible for a free birth certificate, you will need a voucher, which may have been given to you by a shelter in the days before PHCR.  If you are homeless and don’t have a voucher, please go to the DHS booth to check eligibility. booth 7

YourCare Health Plan

A not-for-profit organization, providing free or low-cost health insurance plans, such as Medicaid Managed Care, Child Health Plus, and the Essential Plan. Facilitated Enrollers will share information and assist with enrollment on-site. booth 3

Monroe Plan for Medical Care

The parent company of YourCare Health Plan — dedicated to providing outstanding Health Home Care Management Services.  Care Managers will help those eligible for Medicaid Managed Care, Child Health Plus, and the Essential Plan navigate through their medical, mental, and social service health needs. booth 2

Health Reach

Our medical team will have private rooms for our physicians to treat attendees with acute illnesses, provide health screenings, and management of chronic medical conditions. Our intensive case managers and therapists will be available to consult with attendees and schedule future appointments.  Individuals can sign up with Health Reach to receive ongoing care from our providers at our clinic, our Mobile Medical Unit, or at local homeless shelters that we partner with. We provide services to patients regardless of ability to pay. We will have an insurance enroller on-site to assist patients. booth LL1

His Branches Health Services

We specialize in family medicine, pediatrics, OB care, women’s health, adult and geriatric care, diabetes education, acute and chronic disease management. We have offices in the 19th Ward and Beechwood. We accept Medicaid, Medicare, private insurance and self pay. Stop by and sign up to be a new patient.  Booth LL4

Anthony L. Jordan Health

Activities onsite include patient registration, HIV/Hep C Testing, and Medication-assisted addiction treatment (MAAT). MAAT provides buprenorphine (Suboxone® or Subutex®) treatment to individuals with opioid dependence. This includes people with addiction to heroin or opiate pills like Vicodin® or OxyContin®. We also offer treatment for nicotine dependence and cocaine dependence for those who are receiving buprenorphine treatment. We also offer Naltrexone (Vivitrol), an injection for treatment of alcohol and opioid use disorder. booth LL2

Trillium Health

Our team will provide patient registration, HIV/HEP C testing, information on PrEp (Pre-Exposure Prophylaxis) for HIV, Narcan training, care management services (housing, transportation, food cupboard information, health homes care management services), and insurance navigators. booth LL8

Wegmans School of Pharmacy at St. John Fisher College

We will be providing flu shots to participants aged 9 years and above. booths 25-26

Health Reach Dental Van

In the mobile van outside the arena, we will be able to provide: dental exams with x-rays, dental cleanings and oral hygiene education, fillings and sealants, tooth removal, full and partial dentures, oral cancer screenings and other dental procedures In van at curb outside

Monroe County Department of Public Health

We will educate attendees about the opioid crisis in Monroe County and equip attendees to respond to a suspected opioid overdose by teaching the signs and symptoms of an OD. Will give all homeless attendees a free kit containing the reversal nasal spray medication, Narcan (Naloxone), while supplies last. (Unfortunately, we cannot provide Narcan to volunteers or staff assisting homeless clients. However, all will learn how to get a kit for themselves during the presentation.) Suite 23

We will provide dental education, dental screenings, free toothbrush and toothpaste. We will also schedule dental appointments for guests who don’t have a dental home. booth 28

Cancer Services Program of Monroe County/UR Center for Community Health

We help individuals without health insurance get screenings for breast, cervical, and colorectal cancer at no cost. booth 39

We provide medicaid long-term care services and intensive case management to people with chronic medical and mental health issues. booth 40

Liberty Resources

We are a Care Management Agency connected to a Mental Health Clinic. Care Management is a service for Medicaid recipients who meet the qualifications, who need help with any of the following: Connecting you with health providers such as doctors, dentists, and specialists; Scheduling and keeping medical appointments and making sure you have the follow-up care you need; Ordering, learning about, and taking your medication; Finding available programs, services, housing, and other resources; Applying for or keeping your SSI/SSDI benefits; Completing recertification paperwork, insurance forms, or medical and other referrals. Care Management is a voluntary program, and it doesn’t cost you anything. If you decide to enroll in our program, you can decide to withdraw at any time.  booth 41

Legal Assistance of Western New York, Inc.® (LawNY®) provides free legal aid to people with civil legal problems. Disability Rights New York

Disability Rights New York is the Protection & Advocacy System and Client Assistance Program for persons with disabilities in New York. New York residents with disabilities can contact DRNY for assistance with problems directly related to their disabilities. DRNY investigates complaints and provides direct assistance to individuals with disability-related issues under the terms of its federal grants and its annual goals and priorities. DRNY is committed to enabling those we serve to exercise their own life choices, participate fully in community life, and provide equal opportunity for individuals with disabilities that is free from discrimination, abuse and neglect. booth 15

Rochester Regional Health Chemical Dependency

We provide comprehensive and personalized treatment services for adolescents, adults, and their families. Our highly specialized outpatient programs, inpatient rehabilitation and residential programs help our clients and their families recover from addiction and get their lives back. Open Access/Delphi Rise

Open Access is open 24 hours, 7 days a week. We are a navigation resource to SUD treatment; we provide evaluations, MAT, peer services and outreach. In most situations an individual is connected to a treatment facility within 12-48 hours. booth 46

Al-Anon Family Groups

St. Ann’s Community

St. Ann’s Adult Day Services at Durand Senior Apartments provides quality supervision, personal care and socialization for older adults and people with disabilities. We offer medical and social support services in a comfortable, stimulating home-like setting. Hours of operation are Monday-Friday from 7:30am-5:00pm. Transportation to and from program is provided for ambulatory and wheelchair needs. booth 50

Catholic Charities Community Service

We offer care management service to both children and adults with chronic medical conditions, mental health diagnosis, HIV, and traumatic brain injuries. booth 52

Finger Lakes Area Counseling & Recovery Agency

FLACRA serves individuals and families whose lives are affected by substance use and other health-related disorders. Services include addictions crisis counseling, outpatient services, community residences, supportive living, housing, and care management. Booth 53

Rochester City School District

We protect students’ educational rights when they are experiencing housing instability. We assist with registration/school placement, transportation, food services, clothing, school supplies, case management, bus passes. booth 63

The Salvation Army Genesis House

A 30-day emergency shelter for homeless youth ages 16-20. booth 64

People Inc

We connect families with children with chronic health and/or mental health issues with Care Managers who link them with resources to deal with these issues. Children from 0 to 21 qualify. booth 65

National Council of Jewish Women

We will be giving guests menstrual kits for one month — pads, tampons, and pantiliners. booth 56

Highland Family Planning

Highland Family Planning offers high-quality, low-cost, and completely confidential birth control and reproductive health care to all people, including teens. We offer same-day and walk-in appointments, no matter whether you have health insurance, are under-insured, or uninsured. booth 58

Women’s Initiative Supporting Health

Medical care, mental health/substance use disorder referrals, and trauma informed peer navigation assistance for women re-entering from incarceration or other justice involvement. booth 59

Willow Domestic Violence Center

Willow provides services for survivors of domestic violence, including a 24/7 Hotline, emergency shelter, counseling, court advocacy and prevention education services. Advocates from our Mobile Program will be providing safety planning and brief consultations for those who would like to talk. booth 60

Healthy Baby Network

We work with mothers and families to provide access to care, job training, and housing security to ensure a healthy, stress free pregnancy. Our outreach includes walking the streets and providing access directly to those in need. booth 61

Samaritan Center of Excellence, Inc.

Peer-to-peer support services (life skills, recovery support groups, job readiness training, CPRA/information sessions, housing support, Home and Community Based-Services (HCBS), and family-focused social activities. All services emphasize empowerment and mutual support among justice-involved women and families in recovery from substance use/mental health disorders.  booth57

We help veterans enroll or register for healthcare with the VA and talk to them about accessing benefits they may be entitled to. booth 16

Veterans Administration – VA Homeless Team

The Healthcare for Homeless Veterans team (HCHV) can engage eligible veterans in the community with housing, medical, case management and treatment needs. Within the (HCHV) is the Mobile Outreach Team (MOT) that seeks to engage veterans who are not currently connected to services. The services and referrals can be made within the VA and the community. booth 17

Rochester Vet Center

We provide counseling services to all warzone veterans, spouses, and their families. We provide counseling to all military sexual trauma cases. We provide bereavement counseling to all those who have lost a loved one in the service. booth 19

Soldier On

Soldier On assists homeless veterans in securing permanent housing. Soldier On also works with housed veterans that are at risk of eviction to help prevent being evicted. booth 20

Warrior Salute Veteran Services

WSVS helps veterans with post-traumatic stress disorder, traumatic brain injury and military sexual trauma regain their lives by providing quality clinical and rehabilitative therapies, transitional housing and case management. Staff work one-on-one with veterans, and their families, to help identify their goals, determine the personalized support needed and implement the services and training necessary to foster growth and development as they transition back into the civilian community. Warrior Salute Veteran Services welcomes both male and female veterans as participants.  Outpatient services include individual mental health counseling, group therapy, art therapy, yoga, mindfulness, case management services, and family support. booth 21

Providing free, professional assistance to veterans and their families in obtaining benefits and services earned through military service and provided by the Department of Veterans Affairs (VA) and other agencies of government. booth 22

Veterans Outreach Center Inc.

VOC serves veterans, Guard, Reservists, and active military and their immediate families. All services are free and include: emergency, transitional and supportive housing; case management; employment and training; wellness and recreational services; financial and legal counseling; veterans treatment court; and a Quartermaster’s Club with a food pantry, clothing, and household items. booth 23

We provide housing programs such as supportive living for the CD population, shelter for the homeless, SRO, and apartments in the community. We have a young adult and young mothers’ division. booth 75

The Salvation Army Booth Haven & Hope House

We provide emergency housing for homeless men, women & children at Booth Haven and Hope House. We can help with immediate assistance with any emergency housing needs. Residents are provided three meals a day and case management. booth 76

Open Door Mission

Coed emergency shelter, women and children’s transitional housing, and addiction recovery program for men. booth 77

Catholic Family Center

Safe emergency shelter for men, women, women with children and families; access to health care & social services; onsite case management with additional supportive services to help clients move to safe permanent housing. Sojourner Home at PathStone

211/ LIFE LINE Goodwill of the Finger Lakes

Our 24/7 hotline provides information and referral for human services, along with crisis counseling and suicide intervention. We can provide referrals to services including housing, food, shelter, mental health, substance abuse, employment, education, health related services, criminal justice and legal resources, and other family and community resources. booth 69

Cameron Community Ministries

CCM has a lunch program Monday thru Saturday, in addition to a food pantry. We also have a clothing house where people can shop for clothes and small household items. Plus, we will be opening a teen center in the fall that will complement our after-school program and summer Safe Haven program. The Father’s Heart ROC Ministry

We are a mobile kitchen that goes out Thursday and Friday nights and Saturdays during the day to different areas in the City of Rochester. We provide a hot meal, groceries, clothing, toiletries, and a Gospel message. booth 67

Rochester Construction Training Center

We have a 12-week training program that covers construction skills leading to well-paying jobs.  No academic prerequisites and a criminal record is not an obstacle. booth 89

Pathstone Employment Focused Services

PathStone’s reentry training & employment programs serve individuals who are or have been involved in the criminal justice system. Our programs serve these individuals by providing them with job readiness training, free vocational training, academic support and employment services. Individuals 18+ may be eligible for our programs. booth 90

We will register voters who had a change of address, change of name, change of party (or first time choice of party) or never registered before. Most people on parole are eligible to vote, and we can look up if you are.  All those who have completed parole are eligible.  Volunteers and vendor staff are welcome, as well as guests. booth 72
